What paperwork will I need?

Before calling, make sure you have your insurance information on hand. If you don’t have commercial insurance or Medicaid and are unsure if you qualify, refer to the Eligibility guide on the Medicaid website.

If you don’t have insurance, still call! We offer financial assistance for those who qualify and work with every client to help them receive the care they need.

While there are a few ways to start your inquiry, such as being referred into services or downloading and sending us a referral-registration form, the quickest way to receiving expert-level care at MHP is to give us a call at (303) 443-8500.

During inquiry and registration, our staff will ask you questions regarding your background, insurance, income and state of health. This first step may take 10-15 minutes, so it really helps to have all of your information ready before calling!

Before care begins, an MHP behavioral health provider will meet with you to do an assessment, that helps identify which of our available services will provide the most help. We offer a variety of behavioral health services, such as group therapy and a selection of classes alongside individual therapy.

Once the assessment is done, both you and MHP will have a better idea of what kind of care best suits your needs.
