Care Coordination and Management
Care Coordination and Management deliberately organizes patient care activities and sharing information among all of the participants concerned with a patient’s care to achieve safer and more effective care. This team-based, patient-centered approach is designed to assist patients and their support systems in managing medical conditions more effectively.

Psychiatric Services focus on the diagnosis, treatment and prevention of mental, emotional and behavioral disorders. A psychiatrist is a medical doctor (an M.D. or D.O.) who specializes in mental health, including substance use disorders. Psychiatrists are qualified to assess both the mental and physical aspects of psychological problems.
Psychoeducation refers to the process of providing education and information to those seeking or receiving mental health services, such as people diagnosed with mental health conditions (or life-threatening/terminal illnesses) and their family members.
Life Skills Training
Life Skills Training programs attempt to remedy the difficulties and obstacles in life that having a mental health problem can cause by encouraging independent living. Life skills often have several components: communication and talking; financial awareness and money management; domestic tasks; and personal self‐care. Other life skills include training on coping with stress, shopping for and eating healthy food, knowing the time, taking medication, improving social skills, using public transport and forward planning.
Whole Health Action Management
Whole Health Action Management is a training program and peer support group model developed to encourage increased resiliency, wellness, and self-management of health and behavioral health among people with mental illnesses and substance use disorders. The program provides peer support professionals and volunteers a format for peer support meetings to support one another as they work toward, achieve, and maintain whole health goals.
Shared Decision Making
Shared Decision Making involves a few different characteristics. There must be at least two people, acting as partners, and generally, these are the health care provider and the client. Both partners will take steps in sharing a treatment decision and sharing information about treatment options. The partners will then arrive at consensus regarding the preferred treatment options.
Life Goals Collaborative Care
Life Goals Collaborative Care is an evidence evidence‐based psychosocial intervention designed to improve medical and psychiatric outcomes for persons with mood disorders through personal goal‐setting aligned with wellness and symptom coping strategies and supported through collaborative care.
Targeted Case Management
Targeted Case Management services help individuals with serious and persistent mental illness gain access to medical, social, educational, vocational and other necessary services connected to the person’s mental health needs.

Our team of registered nurses and skilled mental healthcare professionals will work with you and your doctor to help you identify and select the most appropriate treatment options. We have the understanding, expertise and resources to take charge of the situation and coordinate all of your care.
Our Peer Support program is designed to help clients in all stages of recovery. We have first-hand experience with mental illness and the challenges that come with it. Despite those challenges we have come to know the reality of wellness. We believe recovery is possible for everyone and our hope is to reach out to our peers. Our Peer Support Specialists help clients develop their own recovery by recognizing personal strengths and setting goals. As clients, we too have gone through this process and now share a unique empathy and perspective.
Health and Wellness Coaching
Health and Wellness Coachin is open to all adults and includes wellness orientation, wellness coaching, skills classes and workshops covering the nine domains of wellness: physical, social, emotional, spiritual, occupational, cognitive/educational, community involvement, financial and creativity. Most classes are time-limited and there is a conscious intent not to duplicate classes offered in the community, but rather to support people to access community resources.
